Job Description
The Specialized Case Manager position at the Regional Center of the Easy Bay provides enhanced case management for individuals and families with no purchase of service authorizations. Case managers will work to reduce disparities and improve equity in access to services, identify barriers, and partner with RCEB and community agencies. The role focuses on culturally competent care and requires an understanding of diverse communities.
Requirements
- Facilitate the planning process and develop the Individual Program Plan.
- Coordinate and support the implementation of the Individual Program Plan.
- Advocate on behalf of individuals served to protect their rights.
- Monitor and evaluate the services provided.
- Maintain all documentation and contacts.
