Medical Social Worker

Snowline Health's hospice team is seeking a dedicated, compassionate Medical Social Worker to join our mission-driven organization.

Requirements

• Master of Social Work (MSW) degree from a graduate school accredited by the Council on Social Work Education
• Licensed Clinical Social Worker (LCSW) preferred
• Minimum of one (1) year of clinical geriatric experience required; hospice experience preferred
• Valid California driver's license and a reliable, insured vehicle
• Ability to efficiently operate a computer; prior Electronic Health Records (EHR) experience preferred
