Job Description
The Montana Department of Public Health and Human Services has a career opportunity for a Complex Care Coordinator. This position requires researching, developing, and implementing a complex care management initiative for individuals with Alzheimer's disease and related dementias. The position will serve as a lead case manager and report directly to the Executive Director, Medicaid and Health Services Branch.
Requirements
- Knowledge of Alzheimer's disease and related dementias, person-centered care principles, and care models
- Ability to research and interpret federal and state health care regulations
- Skilled in technical writing and verbal communication
- Skilled in managing timelines and managing the logistics of numerous complex cases
