Boswell Transitional Care of Cascadia is seeking a motivated Licensed Practical Nurse (LPN) to provide person-centered nursing care to residents in a community setting. The role involves direct patient care, prevention of illness, and coordination of care with the nursing and health teams. Requirements

• Graduate of accredited LPN School of Nursing.
• Current valid LPN license in the state employed.
• Valid CPR certification.
• Current certification in a clinical specific area (Long-term care, Gerontology, Wound care, IV therapy, Ventilator, Pulmonary & Tracheostomy, Hospice & Palliative care).
• Six months experience in a long-term care environment preferred.
