Physical Therapist - PRN (as needed)

A physical therapist provides services to restore function, improve mobility, relieve pain, and prevent or limit permanent physical disabilities of residents/patients suffering from injuries or disease.

Requirements

• Establishes and administers a treatment plan
• Assesses patient needs and plans for, evaluates and modifies care
• Collaborates with all disciplines to plan and evaluate team goals
• Sets realistic and achievable goals for patients/residents
• Documents and records patient/resident condition and progress
• Educates patients and families in an appropriate therapy method
• Ensures punctuality and regular attendance for assigned shifts
