Job Description
Sun River Health is seeking a Full-time Patient Navigator to support our Haverstraw site. The Patient Navigator identifies individuals and/or members of their families within the target population who are in need of preventive and primary health care services and links them to the services they need.
Requirements
- High school diploma/GED
- 2-4 years of work-related experience
- Bilingual Spanish is preferred
