Job Description
This role focuses on providing crisis intervention, biopsychosocial assessments, individual counseling, and case management services to HIV-positive patients and their families. Social Workers of Adult Day Health Care ensure patients maintain their wellness, connect them with resources, and coordinate mental health and case management referrals. They also perform daily routine tasks and ensure program compliance.
Requirements
- Master’s degree in Social Work required
- CPR/BLS certification
