This role focuses on providing crisis intervention, biopsychosocial assessments, individual counseling, and case management services to HIV-positive patients and their families. Social Workers of Adult Day Health Care ensure patients maintain their wellness, connect them with resources, and coordinate mental health and case management referrals.
