Job Description
The Case Manager supports Clients’ housing stability, self-sufficiency, and community integration through evidence-based practices such as harm reduction, housing first, and trauma-informed care.
Requirements
- Bachelor’s degree preferred
- 2 years' experience working with homeless individuals
- Valid Driver’s license, reliable car, auto insurance, and an acceptable driving record
- Demonstrated knowledge of case management
- Ability to work in a high tolerance Harm Reduction model
