Job Description
The Healthy MOMs Case Manager provides comprehensive case management services to address the opioid epidemic as it affects pregnant women, newborns, and their families.
Requirements
- Five years' experience in substance abuse care and services
- One year of experience working with clients/patients with substance abuse disorders and mental health disorders
- Experience with linking clients/patients up with supportive services in the community
- Bachelor's degree (minimum) in social work, psychology, sociology, counseling, or other related field
- Valid Driver's License
