Job Description

Requirements
- Rounding and managing clinical care of patients at assigned SNF.
- Prescribing medications and treatments for residents at SNFs.
- Providing comprehensive primary care services to patients residing at SNFs.
- Timely documentation of patient care/encounters.
- Availability for phone/secure messaging consultations with facility nursing staff.
- Reliable transportation.
