Hospice - Clinical Educator

Transitions Care is a leading hospice organization that provides 24/7 access to services in the comfort of the patient's home. The Clinical Educator at Transitions Care serves as a key member of the corporate clinical team and is responsible for the development, implementation, and oversight of standardized clinical education and competency programs.

Requirements

• Bachelor’s degree in Nursing (BSN) preferred.
• RN licensure in the primary state of assignment, with ability to obtain in other states.
• C.H.P.N. (Certified Hospice and Palliative Nurse) certification preferred, but not required
• Five (5) + years general nursing preferred.
• Minimum 2 years of hospice experience
• Strong ability to teach, train and educate others
• Ability to work within an interdisciplinary setting
• Demonstrate excellent observation, assessment, nursing judgment and communication skills
• Strong ability to multitask, and prioritize patient needs
• Valid driver's license and reliable transportation
• Must be available to travel >75%
