Job Description
Transitions Hospice is looking for an RN Case Manager to provide compassionate care to patients and their families in the hospice setting.
Requirements
- Active RN license in the state of employment
- Graduate of an accredited school of Nursing
- One (1) + year Hospice, home health or related organization experience
- Compassion for patients and families navigating end-of-life care
- Strong clinical judgment and the ability to work independently in the field
- Excellent communication skills across care teams and families
