Community Health Navigator

The Community Health Navigator is responsible for helping patients and their families navigate and access health and community services and adopt healthy behaviors. They work closely with medical providers to ensure comprehensive and coordinated care, and provide social support and informal counseling to patients and their families.

Requirements

• High School Diploma or its equivalent
• Excellent time management and organizational skills
• Excellent written and verbal communication skills
• Creativity, flexibility, sound judgement, and ability to take initiative
• Excellent interpersonal skills and demonstrated ability to interact professionally with culturally and educationally diverse staff and clients
• Written and oral fluency in Spanish and English required
• Experience working in a multicultural setting
• Basic computer skills (Outlook, Excel, and Powerpoint helpful)
• Ability to initiate and maintain positive working relationships with team members and other organizations
• Positive attitude and open to changing environment
