Case Manager

Veterans Multi-Service Center (VMC) is seeking a compassionate and dedicated Permanent Supportive Housing (PSH) Case Manager to empower chronically homeless individuals and families to achieve and maintain stable housing.

Requirements

• Bachelor's degree in Social Work, Human Services, Psychology, or related field
• 2-3 years of case management experience, preferably with chronically homeless individuals or HUD-funded programs
• Strong understanding of Housing First, trauma-informed care, and harm reduction
• Excellent interpersonal, communication, and organizational skills
• Proficiency in MS Office and HMIS/client management databases
• Valid driver's license and reliable transportation
• Empathy, resilience, and a passion for serving vulnerable populations
