Social Worker

This role involves ensuring patients reach the end of life, honoring their wishes for finances, funeral arrangements, and offering a supportive experience within a dedicated hospice setting.

Requirements

• Ability to travel per job requirements.
• Thorough knowledge of community resources or ability to obtain.
• Knowledge of key psychosocial issues with terminally ill patients and their families.
• Capacity to relate with others and work as part of an interdisciplinary team.
• Excellent listening and communication skills.
• Ability to advocate for patients within the care setting.
