Job Description

This role involves ensuring patients reach the end of life, honoring their wishes for finances, funeral arrangements, and offering a supportive experience within a dedicated hospice setting.
Requirements
- Ability to travel per job requirements.
- Thorough knowledge of community resources or ability to obtain.
- Knowledge of key psychosocial issues with terminally ill patients and their families.
- Capacity to relate with others and work as part of an interdisciplinary team.
- Excellent listening and communication skills.
- Ability to advocate for patients within the care setting.
