Case Manager

We are seeking a Case Manager to join our healthcare team, providing comprehensive and compassionate case management services to clients. The Case Manager will collaborate with healthcare professionals and others to address clients' medical, social, and emotional needs, ensuring they receive the highest quality of care.

Requirements

• Bachelor's degree in Social Work, Psychology, or a related field
• Previous experience in case management preferred
• Valid Government Issued Driver's License required
• Experience working in healthcare and social service systems preferred
• Familiarity with coordinating and managing patient care plans, ensuring comprehensive and integrated services
