Job Description

Requirements
- Perform recertification's, resumptions of care, discharge assessments, and routine client visits.
- Counsel clients and families about nursing and related needs.
- Communicate with the Nurse Case Manager about healthcare provider orders, necessary equipment, incident reports, and changes in client condition.
