Physical Therapist - Home Health

21st Century Home Health Services is seeking a full-time Physical Therapist to provide skilled services to patients in their homes. They are committed to compassionate, empathetic care and have a high customer satisfaction rate, receiving over 10% readmission rates for patients compared to 15%. The role involves independent, supervised patient evaluations and care plan development, as well as working collaboratively with Case Managers and Paraprofessionals.

Requirements

• Current unrestricted CA Physical Therapist License
• Current BLS/CPR Certification
• Valid CA Driver's License
• Knowledge of local community resources
• Knowledge of state and federal homecare regulatory guidelines (preferred)
• Experience home health care (highly preferred)
