2nd Chance Clinic is seeking a dual role case manager and medical staff associate for its Somerset, KY office. The role involves new patient assessments, level of care determination, and documentation. The candidate should have a bachelor's degree in a human services field and 2 years of experience in behavioral health case management.

Requirements

• Bachelor's degree in human services field
• 2 years' experience in behavioral health case management
• Current certification in BLS, non-violent crisis intervention, and suicide intervention
• Excellent oral and written communication skills
• Basic computer skills in word processing, email and Internet use
• Strong interpersonal skills
• Professionalism
• Maintain required continuing education units per state regulations for targeted case managers
• Complete Targeted Case Management Core training and SUD training
