Job Description
This role involves providing personal care, basic health care and practical support to individuals with assessed needs, to enable them to live as independently as possible in their care setting.
Requirements
- Provide personal care, basic health care and practical support to individuals with assessed needs
- Assist with medication and ensure effective communication with staff, service users and external agencies
- Maintain high standards of confidentiality and follow Council policies and objectives
- Work collaboratively with other staff and external agencies to promote the health and wellbeing of service users
- Participate in quality assurance monitoring system and provide input to care planning and assessment
