Personal Advisor

The Personal Advisor will provide direct services to care leavers, managing complex and high-risk cases, and delivering high-quality practice to improve outcomes for young people. The role involves recording activity, completing Pathway Assessments and Plans, and providing individual support and advocacy to young people.

Requirements

• Hold an allocated caseload and be responsible for the provision of direct services to care leavers
• Manage complex and high-risk cases
• Deliver high-quality practice to improve outcomes for young people
• Record all activity on ICS accurately and in a timely manner
• Complete Pathway Assessments and Plans
• Provide individual support and advocacy to young people
• Negotiate referrals to partner agencies
• Work with young people to identify and address their needs, strengths, and risks
• Motivate and promote life quality improvements with young people
• Work with education and training organisations and employers
• Develop and use a network of voluntary, statutory, and community agencies
• Work with parents, carers, and families
• Manage information effectively
• Review and reflect on individual professional practice
• Act as an advocate on behalf of young people
• Develop areas of expertise
• Identify needs, risk, and strengths, and help provide imaginative learning programmes
• Manage a caseload and actively support cases held by social workers
• Be available for supervision and appraisal
• Develop the team, enhancing service quality and provision
