State Home Care Registered Nurse
Graduate of an accredited school of nursing, minimum one year nursing experience, and excellent computer, organizational, and interpersonal skills
Transitions Coach
Requires a bachelor's degree in human services, health, or a management-related field, and a minimum of 2 years of experience in a social service setting
Geriatric Support Service Coordinator
Bachelor's degree in Social Work or related field, 2 years of experience in casework or related activity, ability to work with diverse population, good communication skills, and ability to maintain effective interpersonal relationships
