The Case Manager I provides individual case management to formerly homeless individuals and families utilizing a motivational interviewing, critical time intervention, and stages of change framework. This role involves intake, assessment, goal setting, case notes documentation, housing application processing, crisis management, and referral services. The CM-I collaborates with other team members and supports retention and improved well-being.
