Case Manager I

The Case Manager I provides individual case management to formerly homeless individuals and families utilizing a motivational interviewing, critical time intervention, and stages of change framework. This role involves intake, assessment, goal setting, case notes documentation, housing application processing, crisis management, and referral services. The CM-I collaborates with other team members and supports retention and improved well-being.

Requirements

• Basic Qualifications: Bachelor degree from an accredited college or university, two (2) years of case management/client advocacy services with people who have been homeless and/or have a mental illness
• One (1) year experience utilizing a client tracking database system
• One (1) year experience utilizing evidence-based practices in a social services field
• Demonstrated experience in developing and facilitating life skills groups
• Proficiency with Microsoft programs (Word, Excel, Power Point, Outlook)
• Ability to provide crisis management services
• Experience working in a team-building environment
• Proficiency with the HMIS system
• Valid California driver’s license
• Access to a personal vehicle
