Job Description
Case Manager develops and implements individualized Case Management Plans for youth ages 8 –20 and their families. Provides direct support to members during programming hours and facilitates groups.
Requirements
- Bilingual (English/Spanish) with strong reading and writing proficiency
- Bachelor’s degree (B.A) in Social Services, Psychology or related field
- 1-2 years of case management experience (working with any age group)
- Experience working with youth (Children, Adolescents, and Young Adults)
