Job Description
We are seeking an experienced and detail-oriented Service Coordinator to oversee and manage homecare services for individuals in the Nursing Home Transition & Diversion (NHTD) program.
Requirements
- Prior experience in NHTD, TBI waiver programs, or homecare service coordination.
- Strong knowledge of NYS DOH regulations and Medicaid-funded programs.
- Excellent organizational and communication skills.
- Ability to work independently while coordinating with multiple stakeholders.
