Job Description

The Virtual Outreach Specialist will be the first point of contact for patients to learn about Accompany Health services and will be responsible for scheduling Initial Visits with our care team.
Requirements
- Conduct outbound calls and virtual outreach to patients to provide information and education about Accompany Health services and resources
- Monitor and answer all inbound calls and SMS from patients interested in learning more about our program
- Support patients by answering their questions, addressing their concerns, and providing guidance on how Accompany Health can support their needs
