Job Description
Join Addiction Recovery Care, LLC (ARC) as a Targeted Case Manager and help individuals gain access to needed recovery supports, medical, social, educational, and other services.
Requirements
- Maintain confidentiality and comply with company, state, federal and HIPAA rules and regulations
- Charting for insurance purposes
- Transport clients to appointments when necessary
- Monitor client's emotional well-being and behavior
- Connect client with family and community resources
- Connect client with legal entities when necessary
- Motivate clients toward goal achievement
- Meet with clients weekly
- Help residents with long-term care plan development and educational resources
