Field Nurse Practitioner - Butler County, Ohio

We are seeking a certified Nurse Practitioner to conduct in-home wellness risk adjustment assessments for Medicare members and other at-risk populations in Butler County, Ohio. The role involves conducting comprehensive health evaluations, providing preventive health education, and engaging in care coordination and quality support.

Requirements

• Travel Requirements
• Valid unencumbered NP License for the state of Ohio
• Previous in-home risk assessment experience
• Previous 1099 (PRN) experience
• 3 years patient care experience in primary care/adult/geriatric, EMR
• Access to reliable transportation
• Strong ability to work within our EMR system
• Ability to work independently
• Bilingual is a plus
