Inpatient Care Navigator/Discharge Planner RN or SW, Day Shift, Care Navigation

Adventist HealthCare seeks an Inpatient Care Navigator/Discharge Planner RN or SW to complete comprehensive psycho-social assessments and collaborate with healthcare teams for patient transitions.

Requirements

• Registered Nurse (RN): Graduate of an accredited school of nursing with a minimum of an associate of science degree required, bachelor’s degree preferred, and 1-3 years clinical experience required
• Social Worker (SW): Bachelor of Social Work (BSW) or Master of Social Work (MSW), and minimum of 3 years of experience in diverse clinical settings
• Active American Heart Association Basic Life Support (BLS) certification
• Cerner EMR and All Scripts experience preferred
