Home Health RN Case Manager - Manitowoc

The Home Health RN Case Manager is responsible for managing a caseload of patients in specialty areas, conducting initial assessments, establishing care plans, and providing direct patient care. The role requires excellent communication skills, strong interpersonal skills, and the ability to work with a variety of clients with patience and respect.

Requirements

• Registered Nurse license issued by the state in which the team member practices
• Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association
• A valid drivers license issued by the Division of Motor Vehicles or Driver license issued by the state of Illinois
• Associate's Degree in Nursing
• Typically requires 3 years of experience in clinical nursing
