Job Description
To integrate, coordinate and monitor each person’s residential and comhab-R support programs and plans with a person centered approach.
Requirements
- At least one year of experience working in the field of developmental disabilities
- Doctor of medicine or osteopathy, Registered Nurse, or Bachelor’s degree in a professional category
- Flexibility of schedule to meet job duties/individual needs
- SCIP-R Theory
- Proficiency in Microsoft Word and Excel
- Ability to work independently and reliability in decision making
- Valid NYS Driver’s License
- Ability to adhere to Agency policies including attendance
