Community Health Worker

Akido is rebuilding the healthcare experience from the ground up, focusing on early interventions, world-class care, and human-centered technology. The Community Health Worker will support members with complex medical and social needs, managing their health and wellbeing through education, peer support, and relaying shared experiences.

Requirements

• High School Diploma or equivalent
• Bilingual in English and Spanish (strongly preferred)
• 2+ years of experience with CalAIM populations of focus (people experiencing homelessness, adults with SMI/SUD, adults transitioning from incarceration, adults with complex medical needs)
• Ability to work in a dynamic, outdoors environment
• Ability to work independently as well as part of a team
• Ability to prioritize multiple and competing tasks
• Excellent oral communication skills, as well as strong interpersonal skills
• Ability to use computers to document information into case management software
