Alameda Care Hospice (ACH) is a rapidly growing agency in the San Francisco Bay Area offering patient-centered care in multiple counties. They are seeking a Licensed Vocational Nurse (LVN) to provide skilled services to patients in their homes under the guidance of a Case Manager. The role offers flexible schedules, autonomy, and opportunities for advancement.

Requirements

• Work under the direction of a RN - Case Manager
• Follow the plan of care
• Respected member of the patient care team
• Current CA RN license
• Current BLS/CPR Certification
• Experience working with an interdisciplinary team
• Case Management fundamentals is highly desired
• Self-Directed
