Medical Social Worker (MSW) - Hospice

Alameda Care Hospice (ACH) is a rapidly growing agency providing patient care in San Francisco, San Mateo, Santa Clara, Contra Costa, Alameda and Salona County. They are seeking motivated Medical Social Workers (MSWs) to perform psychosocial assessments, develop care plans, and coordinate services for patients in hospice and palliative care. Requirements

• Master's degree in Social Work from an accredited school of social work
• Current state licensure as a Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW)
• Minimum of 2 years of experience working in hospice or palliative care
• Excellent communication and interpersonal skills
• Ability to work effectively as part of an interdisciplinary team
• Knowledge of community resources and services that can support the needs of hospice patients and their families
