Medical Social Worker (MSW) - Hospice

Alameda Care Hospice (ACH) is a rapidly growing agency in the San Francisco Bay Area providing patient care in multiple counties. They are looking for motivated and compassionate Medical Social Workers to conduct psychosocial assessments, develop care plans, and coordinate services to support patients and families.

Requirements

• Master's degree in Social Work from an accredited school of social work
• Current state licensure as a Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW)
• Minimum of 2 years of experience working in hospice or palliative care
• Excellent communication and interpersonal skills
• Knowledge of community resources and services
• Ability to work effectively as part of an interdisciplinary team
• Compassionate and empathetic care
