Alameda Care Hospice (ACH) is a rapidly growing hospice agency committed to providing compassionate patient care in San Francisco and surrounding counties. They are seeking a Registered Nurse (RN) to become a Hospice Case Manager, responsible for creating and implementing individualized care plans. The role involves assessing patient needs, administering medications, providing care instructions, and coordinating patient and family interactions.

Requirements

• 1+ years Hospice Case Manager Experience
• Current and unrestricted CA RN license
• Current BLS/CPR Certification
• Experience working with an interdisciplinary team
• Case Management fundamentals
