Requirements

• A High School Diploma or GED
• A minimum of 3 years of experience directly working in the health care environment or a community outreach setting
• Community Health Worker (CHW) certificate
• Preferred experience in a managed health care environment, preferably IPA, HMO, or Health Plan
• Preferred experience working with a diverse population
• Bilingual English/Spanish is strongly preferred
