Registered Nurse, Case Manager & Care Coordinator Team Lead FLOAT

We are looking for a Registered Nurse, Case Manager & Care Coordinator Team Lead to manage PACE participants in the ambulatory setting. The ideal candidate has a valid RN license, 1 year of Geriatric Nursing experience, and 1 year of managed care experience.

Requirements

• Current valid License as a Registered Nurse through the California Board of Registered Nursing
• A minimum of 1 year of experience in Geriatric Nursing
• A minimum of 1 year of experience in a managed care setting
