Pediatric Home Health Case Manager, RN

We are seeking a dedicated and compassionate Pediatric RN Case Manager to provide comprehensive case management services to pediatric patients and their families.

Requirements

• Conduct initial assessments of pediatric patients to determine their healthcare needs and develop individualized care plans.
• Coordinate and monitor patient care to ensure timely and appropriate services are provided.
• Collaborate with healthcare providers, including physicians, therapists, and specialists, to facilitate continuity of care.
• Advocate for patients and families to ensure their needs are met and their voices are heard in the healthcare system.
• Educate patients and families about their healthcare options, treatment plans, and resources available to them.
• Maintain accurate and up-to-date documentation of patient interactions, care plans, and progress notes.
• Participate in multidisciplinary team meetings to discuss patient cases and develop comprehensive care plans.
