Job Description
Angel of God Resource Center, Inc. (AOGRC) is seeking a proactive and motivated Case Manager to join their team in Chicago, IL. AOGRC has been serving at-risk youth and adults for over 20 years, providing job training, support services, and opportunities for employment. The role involves establishing and maintaining relationships with participants, developing individualized plans, and advocating for resources.
Requirements
- Identify, recruit and screen participants for programs/services eligibility.
- Complete intake and maintain service plans for each participant.
- Develop IEP (individual Employment Plan) for each participant.
- Interact with participants by establishing a professional relationship.
- Assist and involve/encourage participants in workforce development opportunities.
- Provide needs assessment and refer participants to appropriate services.
- Maintain a positive adult mentor/mentee relationship while reinforcing positive behavior.
- Respond to any crisis and intervene as needed with proper follow up.
