Registered Nurse (RN) Case Manager – Home Care (LHCSA)

The RN Case Manager is responsible for completing initial and ongoing nursing assessments, supervising Home Health Aides/PCAs, developing and updating individualized care plans, and ensuring safe, high-quality care for clients receiving non-skilled home care services.

Requirements

• Associate Degree in Nursing (ASN) required
• Bachelor of Science in Nursing (BSN) preferred
• Active New York State Registered Nurse (RN) license
• Experience in home care, community health, case management, or related setting preferred (LHCSA experience ideal)
• Strong assessment, communication, and documentation skills
• Knowledge of LHCSA regulations, MLTC requirements, care plan development, and supervision standards
• Valid NYS driver's license and reliable transportation
• Ability to work independently, manage a field-based schedule, and maintain professionalism in varied home environments
