Job Description
Provide comprehensive primary care services to Veteran patients, including preventive care, chronic disease management, acute and urgent condition evaluation and treatment, mental health screenings, and team lead within the Patient Aligned Care Team (PACT) model.
Requirements
- Board Certified or Board Eligible in Internal Medicine (ABIM, AOBIM accepted)
- Active, full, and unrestricted license to practice medicine in any U.S. State or Territory
- Active NPI report
- Active DEA license
- Maintain an active AHA BLS
- Must be able to pass a Federal background investigation
- Sit, stand, walk, lift, squat, bend, twist, and reach above shoulders during the work shift
- Lift up to 50 lbs from floor to waist
- Lift up to 20 lbs
- Carry up to 40 lbs a reasonable distance
- Push/pull with 30 lbs of force
