Job Description
The Anthony L. Jordan Health Corporation (Jordan Health) is seeking a Retention Specialist to contribute to the development of strategies to increase unique active patients. The role involves creating systems, analyzing data, and collaborating with quality team members. This position provides an opportunity to make a difference and impact the quality of life of patients in a network of primary care offices across multiple locations in Rochester, NY, and Canandaigua, NY.
Requirements
- Create systems to reach out to patients from different payer groups and who haven't accessed care in over 12 months.
- Analyze data and communicate with payers.
- Facilitate outreach to new patients to reduce barriers to appointments.
- Collaborate with Quality team members and practice managers.
- Optimize clinical quality and patient safety.
