Part Time Community Health Worker

The Community Health Worker will provide follow-up support to Family Connects Nurse Home Visitors, ensure continuity and effectiveness of services, and collaborate with stakeholders. The position requires strong interpersonal skills, passion for community health, and ability to collaborate with various stakeholders.

Requirements

• Part time position
• Monday - Friday, 8:30 - 5:00, occasional evenings and weekends
• High school diploma
• Community Health Worker Certificate preferred
• Recent community Health work or at least two years of working with a healthcare provider or community-based organization in the area
• Reliable transportation
• Valid driver’s license
• Clean driving record
• Valid vehicle insurance
