Job Description
St. Patrick Center is seeking a Mental Health Case Manager to join their Housing, Employment and Recovery Opportunities (HERO) program, providing mental health assessments, counseling, and case management services to adults with serious mental illness and co-occurring substance use challenges.
Requirements
- Conduct comprehensive assessments of program participants' mental illness, physical health, housing status, and other relevant needs
- Provide direct clinical services to clients on an individual and group basis
- Refer clients to community agencies and assist with coordination of services
