Home Health - Physical Therapist

Provides physical therapy services in clients’ homes as prescribed by the physician and offers consultation and education to agency staff. Conducts initial and ongoing evaluations, develops and updates care plans, and coordinates services and plans discharge. Documents evaluations, treatments, and client responses promptly.

Requirements

• Active Colorado PT license
• Graduate of a physical therapy curriculum approved by the American Physical Therapy Association (APTA) or any accrediting body approved by the U.S. Office of Education
• Valid Driver's License & clean Motor Vehicle Record (MVR)
• Experience in Home Health preferred, but not required
