Clinical Program Pharmacist

The company is looking for a Clinical Program Pharmacist to join its team and help develop and maintain its software platform, which helps clinicians provide expert level medication therapy consultation to patients and their providers.

Requirements

• Doctor of Pharmacy (PharmD) from an accredited college/university
• Licensed by a Board of Pharmacy and in good standing
• Completion of a PGY-1 residency, or equivalent experience
• Board certified in a relevant area
• Ability to review and assess the appropriateness of prescriptions with respect to age and disease-state specific considerations
• Knowledge of chronic disease states, such as diabetes, heart failure, COPD, asthma, cardiovascular disease, hypertension, dyslipidemia, HIV, ESRD, anxiety, depression, bipolar disorders, and thought disorders
