The Director of Community Integration plays a key role in resident orientation and transition, leading weekly and monthly family communication, and supporting the ED in ensuring Artis’s recognition program is delivered.

Requirements

• Lead resident orientation and transition
• Collaborate with cross-functional teams to align care plan and service delivery
• Communicate with families at least once per month
• Maintain effective communications with residents, families, and providers
