Field Case Manager - RN - Home Health

We are seeking a Field Case Manager - RN - Home Health to provide skilled nursing care services to patients in their homes. The ideal candidate will have a graduate degree in nursing, a valid RN license, and experience in home health or public health nursing. The job involves planning, organizing, and directing all aspects of nursing care, as well as supervising home health aides and communicating with clients and families.

Requirements
