RN Field Case Manager

The RN Field Case Manager provides skilled nursing care services by planning, organizing, directing, coordinating, and evaluating all aspects of nursing care in accordance with the physician’s plan of care. The role requires a graduate of an accredited school of nursing, a licensed Registered Nurse, and experience in home health or public health nursing. The candidate should possess excellent verbal and written communication skills, administrative, organization, and leadership skills.

Requirements

• A graduate of an accredited school of nursing.
• Must be a licensed Registered Nurse and passed the state board.
• Has experience in home health or public health nursing preferred but not required.
• Knowledgeable about current nursing trends and legislative issues.
• Must possess excellent verbal and written communication skills.
• Excellent administrative, organization, and leadership skills.
